Service Description: Operates a self government and is responsible for the day to day operations of the Poplar Hill First Nation

Responsible for processing Non-Insured Health Benefits Service claims and coordinating medical transportation for community members
  • Provides transportation to medical appointments within the community for members without access to a vehicle
  • Provides transportation and/or assistance from/to Poplar Hill First Nation to medical appointments (doctor, dentist, optometrist, specialist) in surrounding area
  • Submits medical appointment travel, mileage, meal and accommodation requests on behalf of individual
  • Provides assistance with making arrangements for any medical appointments out of town such as booking appointments, hotels, and completing applications for mileage compensation
NOTE: Individuals with small children must have appropriate car seat restraints or transportation will not be provided

Fees: None
Application:
  • All applications must be submitted at least one week prior to scheduled appointment, but preferably 2 weeks prior

  • Exceptions are made for emergencies
Eligibility / Target Population: Members of Poplar Hill First Nation living on the reserve and requiring assistance with Medical Transportation or Non-Insured Health Benefits (NIHB) arrangements
