Service Description:

Responsible for processing Non-Insured Health Benefits Service claims and coordinating medical transportation for community members

Provides transportation to medical appointments within the community for members without access to a vehicle
Provides transportation and/or assistance from/to Poplar Hill First Nation to medical appointments (doctor, dentist, optometrist, specialist) in surrounding area
Submits medical appointment travel, mileage, meal and accommodation requests on behalf of individual
Provides assistance with making arrangements for any medical appointments out of town such as booking appointments, hotels, and completing applications for mileage compensation

NOTE: Individuals with small children must have appropriate car seat restraints or transportation will not be provided

Application:

All applications must be submitted at least one week prior to scheduled appointment, but preferably 2 weeks prior
Exceptions are made for emergencies

Eligibility / Target Population:

Members of Poplar Hill First Nation living on the reserve and requiring assistance with Medical Transportation or Non-Insured Health Benefits (NIHB) arrangements
