Service Description:
|
The Points North Family Health Team includes: a Family Physician, Nurse Practitioner, Registered Nurse, and a Social Worker. Family Health Teams allow physicians and other allied health care providers to work as a team to focus on keeping patients healthy.
Medical care team focused on family and community health- Offers timely access to high quality primary care
- Provides access to other health care professionals within and outside of the Family Health Team
- Coordinates documentation if patient receives care from a member of the team
- Administers various health programs
- Participates in teaching with a regular rotation of medical students and residents from Northern Ontario School of Medicine (NOSM)
- Offers workshops on various topics including but not limited to: Communication and Self Esteem, Coping With Anger and Stress Management
Offers primary health care including but not limited to:- Chronic disease management
- Diabetes program
- Health promotion
- Disease prevention
- Nutritional counselling
- Mental health clinic
- Ear syringing
- Wound care
- Smoking cessation counselling
