Service Description:
|
The Points North Family Health Team includes: a Family Physician, Nurse Practitioner, Registered Nurse, and a Social Worker. Family Health Teams allow physicians and other allied health care providers to work as a team to focus on keeping patients healthy.
Provides mental health services and support- Offers case management services including individual assessment and treatment planning
- Provides supportive mental health counselling to community at large
- Offers mental health support to all programs of the Family Health Team
- Provides referrals to local and out of town community service providers
- Provides individual and public education and information sessions
Also provides:- Advocacy on various committees and boards for increased mental health programs
- Individual bereavement and grief counselling

Eligibility / Target Population:
|
- Individuals 16 years of age and older experiencing mental health issues; do not require a formal diagnosis
- Will provide referrals to required agencies for children younger than 16 years of age
