The Points North Family Health Team includes: a Family Physician, Nurse Practitioner, Registered Nurse, and a Social Worker. Family Health Teams allow physicians and other allied health care providers to work as a team to focus on keeping patients healthy.
Treats acute and chronic illnesses and provides preventative medicineProvides health education, assessment and treatment for both physical and mental health issuesOffers appointments to patients of the clinic whose regular physician is unavailableProvides patients with appointments for urgent, non-life threatening medical issues
