Service Description: Delivers a voluntary parenting support program and promotes healthy child development

Program participants are provided with:
  • Support and education during pregnancy through referral and home visiting as appropriate
  • Follow up via telephone within 48 hours of discharge from hospital
  • Home visits and referrals post birth to offer support to new mothers as required
  • Home visits during child's early years to offer development guidance as required
  • Referrals to other programs and services



Fees: None
Application:
  • Referrals by self, other health care professionals, agencies, or through the Health Unit's Screening process

  • Screening done by Public Health Nurses
Eligibility / Target Population: Pregnant women and families or individuals with children aged 0-6 years
