Service Description:
|
Provides a comprehensive range of acute care in-patient and out-patient services to the residents of Thunder Bay and regional referral services to the residents of Northwestern Ontario
Operates a regional assessment and treatment centre for pre and post-operative care of patients with obesity requiring gastric bypass surgery
Two types of services offered include:
Medical Services
Provides non-medical services, including nutrition and lifestyle counselling, for individuals who cannot or choose not to have surgery
Surgical Services
Performs gastric bypass surgery, however, will refer complex cases to a partner program in Southern Ontario

Application:
|
- Individuals must have their primary care provider request a referral to the clinic through the Ontario Bariatric Registry
- A primary care provider can be a family doctor, a walk-in clinic doctor, or a nurse practitioner
|
|
Eligibility / Target Population:
|
Medical Program
- Individuals 18 years of age or older who are obese; referrals allowed at age 17.5 years of age or older
- Individuals are required to have a body mass index of 30 or higher OR a BMI between 27 and 30 with at least one approved co-morbidity determined by a primary care provider
Surgical Program
- Individuals 18 years of age or older who are obese
- Individuals are required to have a body mass index of 35 or higher OR a BMI between 30 and 34.9 with a least one approved co-morbidity determined by a primary care provider
