Service Description: The Dryden Area Family Health Team includes: Nurse Practitioners, Registered Nurses, Registered Practical Nurses, Registered Dietitians, Occupational Therapist, Registered Pyschotherapist, Patient Care Navigator and Administration
  • Family Health Teams allow physicians and other allied health providers to work as a team to focus on keeping patients healthy
  • The vision calls for physicians, nurse practitioners and other members of the team to practice together in a positive working environment, sharing and benefiting from the complementary knowledge and skills of their colleagues


Provides diabetes education classes (or individual sessions) run by Certified Diabetes Educators, including a Registered Nurse and a Registered Dietitian
  • Classes are offered for 6-7 clients per afternoon
  • Offers individual appointments with a mental health worker and a Physical Therapist

Application: Self referral, Physician, Nurse Practitioner or other healthcare professionals
