Service Description: Provides diabetes education classes (or individual sessions) run by Certified Diabetes Educators, including a Registered Nurse and a Registered Dietitian
  • Classes are offered for 6-7 clients per afternoon
  • Offers individual appointments with a mental health worker and a Physical Therapist

Application: Self referral, Physician, Nurse Practitioner or other healthcare professionals
