Service Description:
  • Operates a fully-accredited, 60-bed acute care facility and provides an array of services
  • Administers the William A 'Bill' George Extended Care Facility


  • Specialized inter-professional teams including Registered Nurse, Social Worker and Dietitian to provide a coordinated approach to diabetes management and treatment
    • Provides case management and screening services
    • Offers basic and advanced foot care and wound care services
    • Offers intensive insulin and medical management and advanced meal planning
    • Provides eye screening and psychosocial support
    • Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes
    • Advises on developing and implementing an approach to care and management of diabetes
    • Assists individuals in finding appropriate services
    • Provides referrals to other health care professionals

    • Physician or Nurse Practitioner referral required
    Eligibility / Target Population: Patients 18 years of age and older with diabetes and related, collective health issues that require more intensive treatment strategies


    Services and programs for any of the following:

    • Persons with Type 1/Type 2 Diabetes

    • Gestational Diabetes

    • Type 1/Type 2 Pregnancy

    • Impaired Glucose Tolerance

    • Impaired Fasting Glucose

    • High Risk persons
