Operates a fully-accredited, 60-bed acute care facility and provides an array of servicesAdministers the William A 'Bill' George Extended Care Facility
Specialized inter-professional teams including Registered Nurse, Social Worker and Dietitian to provide a coordinated approach to diabetes management and treatment- Provides case management and screening services
- Offers basic and advanced foot care and wound care services
- Offers intensive insulin and medical management and advanced meal planning
- Provides eye screening and psychosocial support
- Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes
- Advises on developing and implementing an approach to care and management of diabetes
- Assists individuals in finding appropriate services
- Provides referrals to other health care professionals

Eligibility / Target Population:
|
Patients 18 years of age and older with diabetes and related, collective health issues that require more intensive treatment strategies
Services and programs for any of the following:
- Persons with Type 1/Type 2 Diabetes
- Gestational Diabetes
- Type 1/Type 2 Pregnancy
- Impaired Glucose Tolerance
- Impaired Fasting Glucose
- High Risk persons
