Service Description:

Specialized inter-professional teams including Registered Nurse, Social Worker and Dietitian to provide a coordinated approach to diabetes management and treatment

Provides case management and screening services
Offers basic and advanced foot care and wound care services
Offers intensive insulin and medical management and advanced meal planning
Provides eye screening and psychosocial support
Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes
Advises on developing and implementing an approach to care and management of diabetes
Assists individuals in finding appropriate services
Provides referrals to other health care professionals

Eligibility / Target Population:

Patients 18 years of age and older with diabetes and related, collective health issues that require more intensive treatment strategies

Persons with Type 1/Type 2 Diabetes
Gestational Diabetes
Type 1/Type 2 Pregnancy
Impaired Glucose Tolerance
Impaired Fasting Glucose
High Risk persons
