Service Description: The Dryden Area Family Health Team includes: Nurse Practitioners, Registered Nurses, Registered Practical Nurses, Registered Dietitians, Occupational Therapist, Registered Pyschotherapist, Patient Care Navigator and Administration
  • Family Health Teams allow physicians and other allied health providers to work as a team to focus on keeping patients healthy
  • The vision calls for physicians, nurse practitioners and other members of the team to practice together in a positive working environment, sharing and benefiting from the complementary knowledge and skills of their colleagues


  • Offers blood pressure monitoring and education
  • Educates individuals about high blood pressure (hypertension) to understand the risks and strategies for lowering blood pressure
  • Provides education and information for:
    • Self-monitoring of blood pressure and management through diet, exercise, stress reduction and smoking cessation
    • Cholesterol management
    • Congestive heart failure (CHF)
    • Stroke prevention
    Cardiac Education Program
    • Offers post event cardiac education
    • Includes individual assessment, education and support
    • Develops treatment/education goals for each person and reviews recent event, heart health, blood pressure management and medication use
    • Offers nutrition and healthy lifestyle education with Registered Dietitian
    • Conducts weekly group educational sessions

    Fees: None; Most services covered by OHIP
    Application:
    • All services/programs require registration/appointment but not a referral

    • Nursing Station clinic and Shopper's Drug Mart clinic operate on a drop-in basis


    Cardiac Education Program: Requires a Physician/Nurse Practitioner referral
