Offers blood pressure monitoring and educationEducates individuals about high blood pressure (hypertension) to understand the risks and strategies for lowering blood pressureProvides education and information for:- Self-monitoring of blood pressure and management through diet, exercise, stress reduction and smoking cessation
- Cholesterol management
- Congestive heart failure (CHF)
- Stroke prevention
Cardiac Education Program- Offers post event cardiac education
- Includes individual assessment, education and support
- Develops treatment/education goals for each person and reviews recent event, heart health, blood pressure management and medication use
- Offers nutrition and healthy lifestyle education with Registered Dietitian
- Conducts weekly group educational sessions
