Service Description: The Atikokan Family Health Team includes: Nurse Practitioners, Registered Nurses, Registered Practical Nurses, Dietitian, Mental Health Worker, Pharmacist and Administration
  • Family Health Teams allow physicians and other allied health providers to work as a team to focus on keeping patients healthy
  • The vision calls for physicians, nurse practitioners and other members of the team to practice together in a positive working environment, sharing and benefiting from the complementary knowledge and skills of their colleagues


Educates patients about their medications
  • Utilizes the Ontario Telemedicine Network (OTN)
  • Works by Nurse and patient reviewing all medications with Pharmacist on the OTN who provides medication education, side effects and potential interactions
  • Provides a medication schedule to each patient
  • Records the findings in patient's Electronic Medical Records (EMR) for physician's awareness
Discharge Medication Reconciliation
  • Reviews all medication lists for patients discharged from Atikokan General Hospital (AGH)
  • Records all changes in patient's EMR
  • Refers patients with numerous changes to a 1-hour OTN session with Pharmacist
Also provides:
  • Medication reviews for patients referred to the Falls Prevention program
  • Monitoring of the INR test results; makes phone calls to patients with their new orders and informs them of when it's time for the next blood test
NOTE: Pharmacist makes annual visit to Atikokan Clinic for in-person consultations
