Service Description:

The Atikokan Family Health Team includes: Nurse Practitioners, Registered Nurses, Registered Practical Nurses, Dietitian, Mental Health Worker, Pharmacist and Administration allow physicians and other allied health providers to work as a team to focus on keeping patients healthyThe vision calls for physicians, nurse practitioners and other members of the team to practice together in a positive working environment, sharing and benefiting from the complementary knowledge and skills of their colleagues
Offers a 6-week workshop for individuals living with chronic or long-term health conditions such as diabetes, arthritis, stroke, cancer, colitis, lung disease, fibromyalgia, Parkinson's, heart disease and other conditionsEducates individuals on how to manage their symptoms to become more confident and improve their quality of lifeEach participant receives a free copy of the "Living a Healthy Life with Chronic Conditions" Wellness GuideFacilitated by Registered Nurse and community members living with chronic disease
