Home and Community Care Coordinator
Service Description: Operates a self government and is responsible for the day to day operations of the Biinjitiwaabik Zaaging Anishinaabek (Rocky Bay First Nation) members

  • Political Territorial Organization: Union of Ontario Indians (UOI)
  • Robinson-Superior Treaty


Provides health care services for ill, injured or elderly community members to allow them to remain in their own homes

Provides the following services:
  • Assessment to determine individuals health history and develop a personalized care plan
  • Case management to ensure that care plans are being carried out and that the plan developed is still suitable
  • Home care nursing duties
  • Personal care help with activities such as dressing and bathing
  • Basic foot care
  • Teaching the individual in need to care for themselves
  • Home support with light housekeeping, laundry and meal preparation
  • Respite for family members and caregivers
  • Help with light outdoor/yardwork

Application:
  • Open to referrals from any source

  • Requires prior arrangements for home visits
Eligibility / Target Population: Members of Biinjitiwaabik Zaaging Anishinaabek (Rocky Bay First Nation) who require assistance at home due to illness, injury or age
Languages: English
Language Notes: Ojibway, available upon request
