Service Description: The Atikokan Family Health Team includes: Nurse Practitioners, Registered Nurses, Registered Practical Nurses, Dietitian, Mental Health Worker, Pharmacist and Administration
  • Family Health Teams allow physicians and other allied health providers to work as a team to focus on keeping patients healthy
  • The vision calls for physicians, nurse practitioners and other members of the team to practice together in a positive working environment, sharing and benefiting from the complementary knowledge and skills of their colleagues


Provides mental health services and support
  • Offers individual assessment and treatment planning
  • Provides supportive mental health counselling to community at large
  • Offers mental health support to all programs of the Family Health Team
  • Conducts home visits for individuals who have trouble leaving their home
  • Will accompany clients to appointments if requested
  • Provides referrals to local and out of town community service providers
  • Provides individual and public education and information sessions
Also provides:
  • Advocacy on various committees and boards for increased mental health programs
  • Individual bereavement and grief counselling
  • Coordination of a mindfulness group to teach patients better stress management

Application:
  • Requires booked appointment

  • Must have Physician referral
Eligibility / Target Population:
  • Individuals 13 years of age and older experiencing mental health issues; do not require a formal diagnosis

  • Will provide referrals to required agencies for children younger than 13 years of age
