Service Description: Operates a self government and is responsible for the day to day operations of the Lac La Croix First Nation

Responsible for processing Non-Insured Health Benefits service claims and coordinating medical transportation for community members
  • Submits medical appointment travel, mileage, meal and accommodation requests on behalf of individual
  • Assists with making travel arrangements
  • Maintains community health records, client documentation, receipts and related information
  • Provides transportation to medical appointments within the community and to any medical appointments (doctor, dentist, optometrist) in the towns of Atikokan and Fort Frances

Application: Call Health Centre Reception to book travel on Medical Van

  • Must make arrangements for out-of-town travel at least 2 weeks prior to date of appointment/travel to allow time to process Prior Approval package with Health Canada

  • Must have Confirmation of Appointment (COA) stamped/signed and return to Health Centre after appointment


Must provide the following information when booking:

  • Full name, address and telephone number (Escort name if applicable)

  • Date of birth

  • Status registration number

  • Date, time and address of appointment

  • Doctor's name and specialty

  • Written proof of appointment such as appointment card

  • Mode of transportation
Eligibility / Target Population: Members of Lac La Croix First Nation who require assistance with Non-Insured Health Benefits (NIHB) arrangements and forms, and/or assistance with medical transportation due to age, illness or disability or have no access to a vehicle
