Service Description:

Offers primary health care and complementary, preventative services for the Anishinaabe and Métis populations in and around the Kenora area
Provides affordable accommodation and support for Anishinaabe individuals and families accessing the Lake of the Woods District Hospital
Provides the following services:- Assistance with reserving accommodations
- Client advocacy
- Emotional support
- Language interpretation

Eligibility / Target Population:
|
Anishinaabe individuals and family members accessing the Lake of the Woods District Hospital for medical reasons including:
- Those who are accessing hospital services on an outpatient basis
- Have recently been discharged or
- Are family members of an inpatient who is seriously ill or receiving palliative care
