Eligibility / Target Population:

Anishinaabe individuals and family members accessing the Lake of the Woods District Hospital for medical reasons including:
- Those who are accessing hospital services on an outpatient basis
- Have recently been discharged or
- Are family members of an inpatient who is seriously ill or receiving palliative care
