Operates a 75-bed acute care facility and provides an array of services
Offers holistic practical services for individuals with mental health/addictions issues accessing the Morningstar Centre or Emergency Department to aid in their healing pathFocuses on improving the overall health of individuals and reducing the number of repeat visits to the emergency departmentServices provided include but are not limited to:- Accompaniment to medical appointments
- Assisting clients to obtain identification (ID)
- Assistance with updating Ontario Hospitalization (OHIP) cards
- Completion of housing applications

- Referral can come from Emergency Department or Morningstar Centre staff
- Intake assessment and completion of Inter-agency Intake required
|

Eligibility / Target Population:
|
- Individuals with mental health/addictions issues
- Individuals accessing the Emergency Department at Lake of the Woods Hospital or the Morningstar Centre and requiring aftercare support
