Service Description:

Provides support services for seniors and individuals with specific needs that require assistance to help them remain in their own homes for as long as possible
Provides escorted transportation to medical appointments, shopping, and to participate in various social activities and programsParticipants must meet the eligibility requirements

Application:
|
- Requires completion of application or must be registered with the program
- Must make arrangements at least 48 hours in advance
- Cancellation notice should be given at least 24 hours in advance if possible
|

Eligibility / Target Population:
|
- Individuals 18 years of age and older with specific needs such as disability, illness or injury that require assistance
- Seniors 65 years of age and older
- Medical appointments are a priority
|
