Service Description:

Provides support services for seniors and individuals with specific needs that require assistance to help them remain in their own homes for as long as possible
Provides supervised activities in a group setting for adults with Alzheimer's Disease and other dementia as well as for frail and elderly individualsPrevents premature and inappropriate institutionalizationProvides respite and support for caregivers

Eligibility / Target Population:

- Individuals 18 years of age and older with specific needs such as disability, illness or injury that require assistance
- Seniors 65 years of age and older
