Service Description:
|
Provides support services for seniors and individuals with specific needs that require assistance to help them remain in their own homes for as long as possible
Provides supportive housing and personal support to enable individuals to live independently in their own apartment- Promotes mental and physical health
- Provides activities and outings for program members
Offers assistance from an on-site support worker with maintaining and developing daily living skills including:- Grocery shopping
- Health care coordination
- Healthy meal planning and preparation
- Housekeeping
- Medication reminders
- Personal care
- Security Checks
