Service Description: Provides a variety of rehabilitation services, mental health and addiction services, and long-term care services

  • Offers an out-patient mental health program with a multidisciplinary approach to treating a variety of mental health issues
  • Provides individual counselling
  • Emphasizes skill based training and other integrative educational strategies to help clients achieve their goals

    • Referrals can be made through the Thunder Bay Mental Health Programs Intake Coordinator or a completed Thunder Bay Mental Health Program Referral Form can be submitted through the Access Point Northwest website

    • Referrals are accepted from clients, physicians, and community professionals and agencies

    • Some services require referral directly from a physician or nurse practitioner
