Service Description: The Dryden Area Family Health Team includes: Nurse Practitioners, Registered Nurses, Registered Practical Nurses, Registered Dietitians, Occupational Therapist, Registered Pyschotherapist, Patient Care Navigator and Administration
  • Family Health Teams allow physicians and other allied health providers to work as a team to focus on keeping patients healthy
  • The vision calls for physicians, nurse practitioners and other members of the team to practice together in a positive working environment, sharing and benefiting from the complementary knowledge and skills of their colleagues


Medical care team focused on family and community health
Plays a leadership role in the development of proactive roles for patients in managing their own health through the provision of the following services:
  • Primary prevention
  • Screening
  • Diagnosis
  • Treatment
  • Prevention of recurrence
  • Rehabilitation and support
  • Programs and services are tailored to the patients needs
Offers the following programs/services but not limited to:
  • Chronic Disease Self-Management Program
  • Chronic Pain Self-Management Program
  • Dryden Diabetes Centre
  • Heart Health
  • Mental Health Counselling
Also provides information and educational sessions about:
  • Arthritis and fibromyalgia
  • Asthma and lung health
  • Menopause
  • Nutrition
  • Patient medication assessments with Pharmacist
  • Prenatal appointments, nutrition and activities as well as infant development
  • Smoking cessation
  • Stress reduction and relaxation

Fees: None; Most services covered by OHIP

Eligibility / Target Population: Must be a patient of one of the Family Health Team Physicians
