Service Description:

The Dryden Area Family Health Team includes: Nurse Practitioners, Registered Nurses, Registered Practical Nurses, Registered Dietitians, Occupational Therapist, Registered Pyschotherapist, Patient Care Navigator and Administration. Family Health Teams allow physicians and other allied health providers to work as a team to focus on keeping patients healthy.
Medical care team focused on family and community health Plays a leadership role in the development of proactive roles for patients in managing their own health through the provision of the following services:- Primary prevention
- Screening
- Diagnosis
- Treatment
- Prevention of recurrence
- Rehabilitation and support
- Programs and services are tailored to the patients needs
Offers the following programs/services but not limited to:- Chronic Disease Self-Management Program
- Chronic Pain Self-Management Program
- Dryden Diabetes Centre
- Heart Health
- Mental Health Counselling
Also provides information and educational sessions about:- Arthritis and fibromyalgia
- Asthma and lung health
- Menopause
- Nutrition
- Patient medication assessments with Pharmacist
- Prenatal appointments, nutrition and activities as well as infant development
- Smoking cessation
- Stress reduction and relaxation
