Plays a leadership role in the development of proactive roles for patients in managing their own health through the provision of the following services:- Primary prevention
- Screening
- Diagnosis
- Treatment
- Prevention of recurrence
- Rehabilitation and support
- Programs and services are tailored to the patients needs
Offers the following programs/services but not limited to:- Chronic Disease Self-Management Program
- Chronic Pain Self-Management Program
- Dryden Diabetes Centre
- Heart Health
- Mental Health Counselling
Also provides information and educational sessions about:- Arthritis and fibromyalgia
- Asthma and lung health
- Menopause
- Nutrition
- Patient medication assessments with Pharmacist
- Prenatal appointments, nutrition and activities as well as infant development
- Smoking cessation
- Stress reduction and relaxation
