Workshops are held once a week for 2.5 hours. The number of participants vary at each workshop (between 8-16 participants) and they encourage individuals to attend all sessions to get the most benefit from the program.

Service Description:
  • Offers various workshops for individuals living with chronic or long-term health conditions such as diabetes, arthritis, stroke, cancer, colitis, lung disease, fibromyalgia, Parkinson's, heart disease and other conditions
  • Educates individuals on how to manage their symptoms to become more confident and improve their quality of life
  • Offers workshops online
  • Programs include:
    • Chronic Disease Self-Management Program (6-week program)
    • Diabetes Self-Management Program (6-week program)
    • Chronic Pain Self-Management Program (6-week program)
    • Getting the Most from Your Healthcare Appointment (1.5 hrs)
    • ABC Health Matters workshops (90 mins each):
      • Organizing Care
      • Managing Stress with a Rare or Chronic Disease
      • Healthy Caregiving

    Eligibility / Target Population:
    • Individuals 18 years of age and older with chronic or long-term health conditions and their caregivers

    • Participants must be able to set goals and function in a group setting; for example, the program would not be appropriate for someone with significant dementia or memory impairment, or for someone who would be disruptive in a group
