Service Description:
|
Provides education and services for the prevention, management and treatment of diabetes- Provides case management and screening services
- Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes
- Advises on developing and implementing an approach to care and management of diabetes
- Communicates patients' concerns to family physician
- Assists individuals in finding appropriate services
- Provides referrals to other health care professionals

- New patients may roster with a Physician by completing enrollment form available at office

Eligibility / Target Population:
|
- Patients of the JE Stokes Medical Centre and the Aguasabon Medical Clinic living with diabetes or at-risk of developing diabetes
- Must have a valid Ontario Health Insurance Plan (OHIP) card or other provincial health coverage
