Service Description:

Works in partnership with primary health care providers to offer services for and raise awareness of Hepatitis C virus (HCV) risks, care and treatment

Provides a treatment team made up of HCV Treating Physicians, a Treatment Nurse and a Case Coordinator

Services and support include:

Primarily community based client care
Raising awareness of testing options and access to HCV testing
Access to care and treatment
Access to mental health and addiction counselling
Education and awareness of risk factors and prevention

Health Care or Social Service providers can call for referral form to refer a client or patient for services; forms can be faxed or sent via electronic medical record
Self-referrals also accepted by calling office or toll-free number

Eligibility / Target Population:

First Nations and non-First Nations people living in the Sioux Lookout Area (including 33 remote First Nation communities) who are at risk of acquiring, are living with or affected by the Hepatitis C virus (HCV)
