Service Description:

Ontario Health atHome Medical Assistance in Dying (MAiD) Coordinator provides:- Assistance to support health care providers, patients and family caregivers to access resources and to help connect to clinicians who are willing to provide Medical Assistance in Dying services
- Support for patients to understand the process of MAiD
- Help to make a request for MAiD and to connect to local clinicians who can help patients access palliative care services or medical assistance in dying
