are 17 years of age and younger
live in Ontario
are a member of a household with an income at or below the levels listed on the website

Children are automatically eligible for Healthy Smiles Ontario if they or their family receive:

Ontario Works (OW)
Temporary Care Assistance
Ontario Disability Support Program (ODSP)
Assistance for Children with Severe Disabilities

NOTE:

Children are not automatically enrolled if their family receives Ontario Works and lives in a First Nations community
Children and youth with other dental insurance coverage are no longer automatically excluded
