Service Description: Provides education and services for the prevention, management and treatment of diabetes
  • Provides case management and screening services
  • Coordinates chiropody clinics
  • Works to increase early detection and to reduce the number of avoidable complications that can arise from uncontrolled diabetes
  • Advises on developing and implementing an approach to care and management of diabetes
  • Promotes increased physical activity and knowledge of healthy eating
  • Provides workshops, presentations and hands-on training for cultural activities and traditions
  • Assists individuals in finding appropriate services
  • Provides referrals to other health care professionals

Eligibility / Target Population: Members of Wabigoon Lake Ojibway Nation living with diabetes or at-risk
