Service Description: Supports diabetes management clinics in Treaty 3 communities to assist individuals in understanding and managing their diabetes
  • Provides diabetes resources and educational tools to communities
  • Offers education and self-management support to fit an individual's lifestyle
  • Provides access to Registered Nurses and Registered Dietitians
  • Maintains a library including many diabetes resources available upon request
  • Provides an online list of health centres and diabetes supports across Treaty 3 territory

Eligibility / Target Population:
  • Members of First Nation communities in the Treaty #3 area

  • Individuals with pre-diabetes, Type 1 and Type 2 diabetes
