Service Description: Provides services that promote physical and mental health for community members
  • Facilitates and coordinates visits from specialists including medical doctors from Marathon Family Health Team, dietitians, diabetes educators, traditional healers and dental hygienists
  • Provides minor treatment through community nursing and health team staff
  • Provides health awareness, education and prevention workshops and seminars
  • Coordinates clinics in areas that may include Diabetes, Immunization, Sexual Health and Communicable Diseases
  • Coordinates appointments for foot care sessions through resident Sisters
  • Oversees the community gardens project
  • Arranges various health activities for community members including walking clubs, healthy weight sessions, fitness boot camps, smoking cessation presentations, intramural sports activities and winter activities
  • Identifies and attempts to correct health hazards
Addresses individual health care needs by providing:
  • Intake services to determine health care needs of individual
  • Referrals to appropriate health care providers based on information in the intake assessment
  • Advocacy on behalf of individual with other health professionals
  • Coordination of one-on-one prenatal care with the community health nurse
  • Home visits to the sick, elderly, new mothers and persons with health care needs
  • Assistance with coordinating local and long distance travel arrangements for medical appointments
