Service Description:

Supports for individuals living with prediabetes or diabetes- Provides education and services for the prevention, management and treatment of diabetes
- Incorporates both traditional and contemporary healthy living services focused on reducing the risk of developing Type 2 diabetes
- Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes
Collaborates with other primary care team members to offer:- Case management and screening services
- Dental screening
- Diabetes and preventative education
- Foot care and chiropody services
- Insulin adjustments
- Medical treatment
- Nutritional counselling
- Referrals to other health care professionals
- Regular bloodwork
- Retinal screening
- Traditional healing services
Services are delivered through:- Individual counselling
- Couples and family sessions
- Group sessions
- Community workshops
