Service Description: Provides education and services for the prevention, management and treatment of diabetes
  • Provides case management and screening services
  • Offers basic foot care services
  • Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes
  • Advises on developing and implementing an approach to care and management of diabetes
  • Assists individuals in finding appropriate services
  • Provides referrals to other health care professionals and the Northern Diabetes Health Network
  • Coordinates education and awareness activities for Health Centre staff such as healthy breakfasts, group walking events and health-related videos
