Service Description: Provides culturally appropriate and holistic support services and programs for people with physical disabilities, serious health issues, or those who are frail and/or elderly
Programs and services can include the following:
  • Afternoons for socialization, congregate meals and activities such as Native crafts
  • Deliveries to clients' homes of items such as prescriptions and groceries
  • Accompaniment to appointments and errands
  • Assistance with various tasks such as letter writing and completing forms and documents
  • Weekly friendly visits from support worker in individual's home upon request and calls to clients' homes to check on them and to determine pending appointments
  • Transportation on an as needed basis to all events at the Centre, the hospital or clinic and for errands around town including grocery shopping
  • Transportation for out of town specialist appointments when no other arrangements are available
  • Provision of bus passes and taxi fare and arrangements when required
  • Group walking program at the recreation centre, indoors during winter months and outside during the summer
  • Monthly foot spa day in conjunction with a sharing circle

Transportation Program - Must call a minimum of 24 hours ahead to make arrangements
Eligibility / Target Population: Open to all people 40 years of age and older with a focus on off-reserve Indigenous individuals with physical disabilities, serious health issues, or those who are frail and/or elderly
Transportation Program - Must be a client of the program * Priority given for medical appointments
