Service Description:
|
Provides culturally appropriate and holistic long term support services and programs including the following:- Access to community supports
- Assistance to coordinate medical appointments
- Congregate dining
- Cultural programs
- Home visits
- Physical fitness
- Referrals and advocacy
- Social activities
Transportation Program- Provides transportation on an as needed basis to all events at the Centre, the hospital or clinic and for errands around town including grocery shopping
- Arranges transportation for out of town specialist appointments when no other arrangements are available

- Urban Indigenous people 50 years of age and older
- Individuals who are disabled, chronically ill and/or have special needs
