Service Description:
  • Provides support to youth with developmental disabilities to create a transitional plan for life after graduating from high school
  • Assists families to identify and access resources through transitional planning is school partnership and into community life
  • Assists with applications and interviews such as Developmental Services Ontario (DSO) housing, passport, Ontario Disability Support Pension and access to adult services, Clinical/Surrey Place
  • Offers resource planning for accessing further education, employment, training opportunities and community resources

    Eligibility / Target Population:
    • Young adults 18 years of age and and their families

    • An application for planning purposes is available for youth under the age of 18 years
