Service Description:

Offers a diabetes management and prevention program to assist individuals in understanding and managing their illness and to learn prevention measures

Provides both adult and pediatric services
Offers individual and group educational sessions and health promotion activities
Accepts walk-in patients

Programs and services include:

Access to Traditional programs and medical interpreters
Basic and advanced foot care/assessment
Client support groups
Community screening
Cooking classes
Exercise programs
Grocery tours
Medication counselling and insulin teaching
Presentations to the public and to health care providers
School programs and visits

Program services are provided through:

Dietitian (Adult and Pediatric)
Exercise therapist
Foot care provider
Nurse
Wellness counsellor

Eligibility / Target Population:

Adult services for individuals 18 years of age and older
Pediatric services for children and youth 17 years of age and younger

Services and programs for any of the following:

Persons with Type 1/Type 2 Diabetes
Gestational Diabetes
Type 1/Type 2 Pregnancy
Impaired Glucose Tolerance
Impaired Fasting Glucose
High Risk persons
