Responsible for providing a variety of specialized community geriatric servicesPromotes issues surrounding geriatric mental healthProvides community outreach and presentationsProgram services include:- Cognitive screening and assessment
- Geriatric counselling or therapy
- Care and treatment planning
- Follow up, referral and advocacy services
- In-home support and education to caregivers
Geriatric Psychiatry
Provides assessments and case consultations via videoconferencing with the Toronto Baycrest Centre for Geriatric Care
Psychogeriatric Resource Program
Provides education and support in long term care facilities to mental health clients and staff

Eligibility / Target Population:

- Individuals 65 years of age and older
- Must have a diagnosis of mental illness, Alzheimer's Disease or a related dementia
