Service Description:
  • Delivers centrally coordinated, community-based programs and services to Métis Communities in Ontario
  • Support services are client-driven, family-centered and promote holistic care

  • Programs focus on:
    • Healthy parenting
    • Early childhood development
    • Chronic disease management
    • Healthy aging
    • Independent living
    • Prevention of addictions and problem gambling
    • Mental wellness

    Ontario Telemedicine Network
    • Provides video-conferencing suites on-site which use the Ontario Telemedicine Network (OTN)
    • Allows psychiatrists and other health care specialists to have a remote connection to patients and clients
    • Reduces the need to travel to receive care

    Canadian Prenatal / Postnatal Nutrition Program (CPNP) - For Aboriginal children 0-6 months of age and their families * focus on the health and well-being of off-reserve Aboriginal mothers and babies * address issues of prenatal, infant, and child nutrition and development, parenting and care giving skills

    Community Wellness Worker (CWW) Program -
    Implements client-driven services to reduce family violence and the promotion of healthy lifestyle through culture-based programming

    Home and Community Care -
    Health services provided in a home or community setting for older adults, persons with disabilities, or injured persons * includes nursing care, personal support, foot care, homemaking, meal preparation, and respite care

    Medical Transportation Program -
    Coordinates medical transportation services and assists with medical appointments in other communities

    Mental Health Counselling -
    One-on-one, couple, and family counselling * mental health screening and assessments * short-term and long-term counselling * confidential

    MNO (Métis Nation of Ontario) Healthy Babies Healthy Children -
    Supports for pregnant women and families with children 0-6 years * home visits by nurses and other home visitors help expectant and new parents with having a healthy pregnancy and birth, breastfeeding, nutrition, child development, and being a parent
