Service Description:

Operates as a community-based organization offering primary health care and health promotion programs in areas within the district
Offers services for individuals and families who live with sickle cell disease that include assistance to navigate health needs, manage pain, access resources and help build strong support systems through education, advocacy, and collaboration.
Services include:
- Clinical care and medical management
- Medication support
- Care referrals
- Support groups and peer networks
- Health education
- Facilitation for crisis and emergency support services
