Service Description:
|
A rural health care organization that includes the Atikokan General Hospital, Atikokan Family Health Team and Atikokan Community Counselling to provide a range of health care and community services to residents in the Town of Atikokan and surrounding areas in the Rainy River District
NOTE: Do not call the Atikokan General Hospital Emergency Department seeking medical advice. Due to legal reasons the emergency department staff are unable to provide advice or information over the telephone
Provides education and services for the prevention, management, and treatment of diabetes- Provides case management and screening services
- Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes
- Advises on developing and implementing an approach to care and management of diabetes
- Communicates patients' concerns to family physician
- Assists individuals in finding appropriate services
- Provides referrals to other health care professionals

Individuals diagnosed with either type 1 or 2 diabetes, pre-diabetes, gestational diabetes or those who may be at risk of developing diabetes
