Provides patient and family centred nursing care in clincs, homes, schools, workplaces and long term care facilitiesCollaborates with patient and health care team in accordance with best practicesTeaches patients and family members how to self-manage health conditions to become independent in patient's own careServices offered can include but are not limited to:- Wound care
- Palliative care
- IV therapy
- Chronic disease management
- Dialysis
- Care of medically fragile children
