Service Description:	A health care hub that provides a variety of programs and services to help seniors and individuals with a disability to live independently Delivers meals to individuals who are not able to purchase or prepare their own meals Coordinates with the hospital to deliver pre-made meals to individual's homes including those on special diets Meals are delivered by volunteers who provide regular social contact and check on the health and safety of the client
