The program is intended to be a last resort option when no other public or private funding is available
No home or vehicle may be purchased using the funding provided by the HVMP
Funding or reimbursement cannot be approved for costs incurred prior to the approval of the application
Applicants can access funding one time only for home modifications, except in special circumstances and may access the program for vehicle funding once in 10 years, unless there has been a substantial change in the person's disability
Funding will not be provided to applicants who choose to move from accessible to inaccessible accommodations without an appropriate reason for the move

Service Description:

To maximize the independence, personal empowerment, and community participation of people with disabilities

Provides financial assistance in making modifications to homes and/or vehicles for people with physical disabilities

Fees:

Program - client may have to contribute towards the modifications if gross annual income exceeds $35, 000

Eligibility / Target Population:

Residents of Ontario who have an ongoing or recurring disability/impairment that is expected to last one year or longer
Disability/impairment must impede mobility and results in substantial restrictions to a person's activities of daily living, such as personal care
