Service Description: Operates a self government and is responsible for the day to day operations of the Shoal Lake 40 First Nation

Provides services and assistance with the registration system of all issues pertaining to membership
  • Responsible for the implementation of the Membership Code and registering all life events that affect the Indian Registry (births, deaths, marriages, divorces)
  • Informs community leadership of current and evolving Indian status changes and issues
  • Maintains updated mailing/contact information for members and population statistics
  • Supplies forms, provides guidance for filling out applications and offers assistance in mailing to applicable departments
Provides assistance in the following matters:
  • Birth Certificate Application
  • Application for Registration of a Minor Child
  • Membership Application
  • Registration of a Minor Child Under the Indian Act
  • Registration of an Adult Under the Indian Act
  • Secure Certificate of Indian Status (SCIS)
  • Statement of Live Birth
