Service Description: The Dryden Area Family Health Team includes: Nurse Practitioners, Registered Nurses, Registered Practical Nurses, Registered Dietitians, Occupational Therapist, Registered Pyschotherapist, Patient Care Navigator and Administration
  • Family Health Teams allow physicians and other allied health providers to work as a team to focus on keeping patients healthy
  • The vision calls for physicians, nurse practitioners and other members of the team to practice together in a positive working environment, sharing and benefiting from the complementary knowledge and skills of their colleagues


Provides assistance for patients to work through the healthcare system to receive the needed care and treatment
Assistance can be provided for long and short-term care in the following areas:
  • Applications to financial aid
  • Funding applications
  • Housing and long-term care
  • Medical case management
  • Sorting out complex cases

Fees: None; Most services covered by OHIP

Eligibility / Target Population: Patients of the FHT who require guidance through the healthcare system
